Parent or guardians sign
consent forms for their child to provide information in
surveys for purposes of program evaluation, and for his
or her participation in program activities such as field
trips, sex education, and mentor and internship
opportunities. Parents must also sign the
Participant Application form that describes the ongoing
parental support requirements and review of the daily
goals for their children stated in the participant
Behavior contract that their child agrees to.
